ABSTRACT

PURPOSE: To determine whether patient factors (pre-fitting patient's data) before reverse-geometry lens (RGL) use could be predicting factors for the increase in visual acuity after RGL use. METHODS: The authors reviewed out patient records of 805 eyes of 423 patients wearing RGLs from March 2003 to May 2009. The relationship between patient factors on UVA before wearing a RGL examined at follow-ups and the IVA after wearing a RGL were analyzed. RESULTS: The spherical refractive errors and the cylindrical refractive errors showed statistically significant results (p 0.05). CONCLUSIONS: The refractive error and the corneal astigmatism may be strong predictors of UVA and IVA after RGL use.

ABSTRACT

Motor neuropathy of a lower extremity is well recognized potential complication of procedures performed on patients in a lithotomy position. Mechanisms of nerve injury are unclear but the incidence of perioperative nerve injuries can be reduced if anesthetists are aware of their causes and pathophysiolgies. It is important to note that reduced duration in lithotomy position may reduce the risk of lower extremity neuropathies. We experienced two case of common peroneal nerve palsy after lithotomy positioning. Diagnosis was based on history, a clinical examination and electrophysiologic studies. A neurologic examination revealed hypersthesia over the dorsum of the left foot with inability to perform active dorsiflexion. Electrophysiologic studies showed delayed latency and low amplitude of nerve action potential.

ABSTRACT

BACKGROUND: Arthroscopic shoulder surgery is often associated with severe postoperative pain. It is important to control pain in this setting, not only to improve the patient's well-being but also to facilitate rehabilitation. The aim of this study was to investigate the efficacy of a continuous suprascapular nerve block for pain relief after arthroscopic shoulder surgery. METHODS: Forty patients (20 in each group) scheduled for elective arthroscopic shoulder surgery received a suprascapular nerve block and a catheter was introduced before surgery. The patients were received standardized general anesthesia. After surgery, a single bolus of normal saline (Group I) or 0.2% ropivacaine (Group II) 6 ml was injected through a catheter in each group. All patients received either a continuous infusion of normal saline (Group I) or a continuous infusion of 0.2% ropivacaine (Group II) through the catheter at a rate of 3 ml/hr plus a bolus of 3 ml with a lock out time of 30 min. Pain relief was assessed at 2, 4, 8, 12, 24 hours using visual analog scale (VAS) and verbal pain scores (VPS). RESULTS: VAS and VPS were lower in the ropivacaine group (Group II) than in the normal saline group (Group I). There were no complication in either group. CONCLUSIONS: Continuous suprascapular nerve block using 0.2% ropivacaine is a safe and efficacious treatment for postoperative shoulder pain.

ABSTRACT

In nonsystemic vasculitic neuropathy, clinically only nerves are affected : there are no or few constitutional symptoms or serological abnormalities. The clinical and pathological features are those of an ischemic neuropathy caused by a necrotizing vasculitis of small arterioles. This is a case report of nonsystemic vasculitic neuropathy in a 22-year-old male who has the distal symmetric poIyneuropathy as a symptom of localized vsaculitis. The sural nerve biopsy showed a few myelin digestion chamber suggesting axonal degeneration. The patient was treated with steroid daily with gradual clinical and electorophysiological improvement.

ABSTRACT

Rhus has been known to one of the most frequent plants which produce allergic contact dermatitis. Allergenic component of thus is urushiol which is composed of catechols with a 15-carbon-atom side-chain. Systemic administration can produced hematogenous contact dermatitis and also severe systemic reactions such as drowsiness, convulsion, nausea, vomiting, glomerulonephritis and urticaria. We experienced a case of right anterior and posterior watershed cerebral infarction, generalized urticaria and allergic contact dermatitis following ingestion of urushiol.

ABSTRACT

Adrenoleukodystrophy is an inborn error of metabolism characterized by adrenal insufficiency and progressive demyelmation of brain white matter and peripheral nerves. Authors experienced three cases of adrenoleukodystrophy in a 7 year old boy, a 17 and a 210 year old males that were diagnosed by increased plasma content of very long chain fatty acid(VLCFA). The clinical symptoms include seizure, visual impairment, and hemiparesis. Two cases showed typical radiological findings and sural nerve biopsy was performed in one.

ABSTRACT

Primary or secondary IOL implantation after cataract extraction was performed in 41 eyes of 36 lepromatous leprosy patients, who showed negative skin test, from October 1990 to April 1992 at Chilgok Catholic Skin Clinic. Mean follow-up period was 8.3 months. Visual improvement was achieved in 39 eyes (95.1%), including 25 eyes (61.0%) of corrected vision better than 0.5. There was no serious complication induced by IOL implantation. But photophobia was complained in 18 eyes (66.7%) of 27 iridectomized eyes. We think that we may as well implant IOL after cataract extraction in lepromatous leprosy patients with complicated cataract if there is no active inflammatiQn in anterior chamber, guarded iridectomy is performed and it is possible to follow up considerably.

ABSTRACT

Recently, with the development of ophthalmic surgical technique, many of aphakic patients want secondary implantation of intraocular lens. In aphakic eye without posterior capsule, implantation of posterior chamber lens can be performed by sulcus fixation of lens with prolene suture. But sulcus fixation technique has some complications as iP~rmanent fistula, rotation or tilting of the optics, hyphema, endophthalmitis and so on. In this study, 24 aphakic eyes, 22 patients implanted withl itis claw lens were compared with 13 aphakic eyes, 13 patients implanted with sulcus fixated posterior chamber IOLs. In 24 eyes implanted with iris claw lens, 18 eyes (75%) had a postoperative visual acuity of 20/40 or better and cystoid macular edema, intraocular lens dislocation were observed on each 2 eyes (8.3%) and iridodialysis on 1 eye (4.1 %). In 13 eyes implanted with sulcus fixed PCLs, 12 eyes (92%) had a postoperative visual acuity of 20/40 or better, and cystoid macular edema, vitreous hemorrhage, acute iridocyclitis and glaucoma were observed in each 1 case (7.7%). In aphakic eyes without posterior capsular support, iris claw lens will be a alternative method of intraocular lens implantation.

Although drug-induced transient myopia has been reported to be caused by the use of several drugs, it is very rare and the causes are still uncertain. A considerable literature has accumulated relating the occasional occurrence of myopia in patients receiving carbonic anhydrase inhibitors, especially acetazolamide. However the authors could not find any literature concerning transient myopia induced by dichlorphenamide. The authors reporthed here a case of transient myopia after dichlorphenamide in a patient with serous detachment of the macula with a review of the literatures.
